Our Goals
Network alignment
Participants include independently owned and Children’s Minnesota owned practices along the entire care continuum of primary, specialty and hospital-based care. Recognizing the breadth and depth of our reach to pediatric patients in this community, we are mutually accountable and aligned toward the goal of highest quality, cost-effective care.
Performance program
Children’s Health Network manages a robust performance improvement program, designed by our physician leadership, that focuses on clinical outcomes, best practice guidelines, cost of care and utilization. We partner with our member clinics to continually elevate these metrics to ensure best practice standards and outcomes for children.
Data analytics
We use a robust data set to compile and analyze quality and cost opportunities across our network. This data is shared with our member clinics to equip them with patient-specific engagement opportunities to improve patient care and cost.
Value-based care
The network works with our clinics to engage commercial and public payers in contract arrangements that prioritize pediatrics and create opportunities for value-focused reimbursement.
Guidance & support
The CHN team provides support to member clinics on operational issues, including policy updates, referral management and coding guidance.
Contact center
